On your first visit to our office, we ask that you bring:

1. Photo identification
2. Insurance card
3. Complete list of medications you’re currently taking, along with dosage and your prescribing physician’s name
4. Bariatric surgery patients must watch our online weight loss surgery seminar before their consultation.

If you would like to complete our new patient paperwork before your appointment, please print the New Patient forms and bring them with you.
